Oldham has a new COVID Assessment Hub

The COVID Assessment Hub is open 24/7 and located at the Integrated Care Centre (ICC) - New Radcliffe Street, Oldham OL1 1NL. The Hub has been created for people who have possible symptoms of coronavirus and are unwell, but do not need a 999 response.

The Hub allows us to provide telephone, video and face-to-face consultations and arrange GP home visits when required and appropriate. This lowers the risk to patients and staff as they will be able to be seen by primary care in a safe and dedicated environment.

New Patient Registration

New patients need to attend surgery to join our list. If you have a medical card please bring this with you, otherwise you will need to complete a GMS1 form at our reception. Some new patients are required to attend a new patient check with one of our nurses. A suitable appointment will be made when registering.

Medical treatment is available from the date of registration. Please contact reception for further information. 

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending any information/forms you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
